In order to protect our customers' exclusive pricing, identification is required of all purchasers. State law also requires that our customers supply us a current copy of their state issued Resale Certificate or Resale Permit ID. Once you complete and submit this form, you will receive a confirmation e-mail with instructions on what to do next, along with a blanket resale certificate / permit request form.
